MD Connected is changing the way Canadians access healthcare. Our purpose is to serve all Canadians, whether they live in remote communities of urban centres, by connecting them to our network of medical practitioners through advanced telemedicine technologies.

Our Story

MD Connected is one of the fastest growing healthcare
providers in Canada with an outstanding reputation for
patient and partner relations.

2017

In 2017, MD Connected launched as a telemedicine service provider focused on rural communities in Ontario.

2018

At the onset of the COVID pandemic, MD Connected quickly adapted to provide remote virtual visits to all Ontarians.

2019

MD Connected expanded its medical team to service Alberta, Manitoba, and British Columbia.

2020

Over the years, MD Connected solidified partnerships with some of the most respected and trusted companies in Canada.

2021

MD Connected enters into an equity partnership with ELNA Medical to expand access to virtual and in-person care.

Doctors Queenette Asuquo and Judy Peter's are fantastic doctors! They are both detailed, compassionate, and care about their patients! Amy, the Nurse is outstanding! She is very caring, professional and detail oriented. A definate asset to your doctor's and the clinic! The young gal who sometimes works on the reception desk does not go above and beyond in helping clients. Having said all of this, the clinic does not run smoothly. Amy cannot do the role of both Admin and Nurse. I have spent hours observing the clinic because there is no direct phone number to reach anyone. The 1-800 number doesn't work, calls are not returned. I go in person for everything. As far as I can tell, there are absolutely no procedures in place for how things get done. Two pharmacies have told me to get hard copy prescription forms because they know that their faxes go into cyber space and are never acknowledged. I could provide more thoughts on my observations, but don't want to take up space here.

University Health Insurance Plan (UHIP)

1. Have your UHIP Member ID and Policy # available so the MD Connected patient care team can verify your coverage eligibility.
2. Please complete the UHIP claim authorization form provided below and email it to admin@mdconnected.ca.

Guard.Me International Insurance

1. Have your Guard.Me policy card ready so the MD Connected patient care team can verify your coverage eligibility.
2. Please complete the MD Connected authorization form provided below and email it to admin@mdconnected.ca.

MSH International (Intrepid 24/7)

1. When using this service, you or your guardian can call Intrepid 24/7 prior to seeking an appointment, and their customer care team will direct you to a contracted clinic.
2. Intrepid 24/7 will fax over the pre-authorization forms. If the forms have not been received by MD Connected you must call the insurer to fax the Verification of Benefits to us.
3. You or your guardian must present your Intrepid 24/7 policy card upon arrival so that our patient care team can verify that you have eligible coverage and sign the authorization forms.

Blue Cross (IFH Refugee)

1. You must provide one of the below forms. As long as you have coverage, which can be validated by an MD Connected customer care team member, your visit can proceed.
a. A refugee protection claimant document (RPCD) with photo.
b. An interim federal health program certificate (IFHC).
